**Full Time Social Worker Rural Generalist - Mount Gambier Hospital****Join SA Health's Dynamic Team in South Australia's Beautiful South East**About This Exciting OpportunityAn opportunity is available to work for Country Health Connect, where you will provide a broad range of services to the Mount Gambier Hospital. This new and dynamic full-time, ongoing position offers a unique split role servicing both our Emergency Department/Short Stay Unit and various hospital wards. As an Allied Health Professional Level 2, you'll work within our Country Health Connect unit, providing comprehensive social work services across diverse healthcare settings.As a member of a multi-professional team, including health professionals and service providers from other sectors, the Rural Generalist Social Worker utilises a combination of preventative, early intervention, treatment / therapy and evaluation approaches.The Rural Generalist Social Worker may be required to work within Subacute services, in which case the Social Worker will adopt a rehabilitative approach to service provision, as defined in the Statewide Rehabilitation Clinical Network Rehabilitation Generic Core Competencies Framework. Services will focus on improving health outcomes, functional capacity and quality of life, in accordance with clinical guidelines and models of care.Key responsibilities:
  • Support consumers through the Patient Journey, providing effective assessment and triage, timely referrals, accurate information, co-ordinated care and prompt follow up
  • Apply clinical skills to a broad scope of practice, delivering services which promote wellness, re-ablement and empowerment appropriate to the local rural context
  • Contribute professional leadership in the application of clinical protocols and standards within the multi-disciplinary team and health unit / LCLHN

ABOUT YOU
  • You will bring experience in primary and acute health care settings, Social Work assessments, counselling, and working in a multi-disciplinary team.
  • Must hold a recognised qualification within the Social Work profession, and be eligible for practicing membership of the Australian Association of Social Workers. It is desirable to participate in the Australian Association of Social Workers accredited continuing professional development program as a requirement of a self-regulated profession.
