Permanent Part-Time Position - 32 hours per fortnightWe are excited to offer a Permanent Part-Time position for a Level 3 Community Support Worker for our Housing & Accommodation Support Initiative (HASI) Program, based in Gosford, Central Coast. Usual hours of work vary from Monday to Sunday between 6am to 8pm with a 24/7 rotating roster.This is an identified position under Section 14 of the Anti-Discrimination Act 1977 (NSW), and we strongly encourage applications from Aboriginal and Torres Strait Islander people.New Horizons is a great place to work and grow. This position supports the delivery of community-based support in the Mental health space.The Day to Day
Providing support to Participants in the mental health community outreach services, to improve their independence by assisting with living skills, transport, social activities, medication runs as well as other supports as required.
Keeping customer records up to date using online systems.
Developing and maintaining professional relationships with people, families, caregivers and key stakeholders.
Complete risk assessments, support plans and other required documents in a timely manner.You are

Invested in providing the highest quality in Mental Health services to the community.
Competent in working with people living with Mental Health.What you need to have
Appropriate certificate level qualification in Mental Health or Community Services, OR equivalent relevant experience in the human services industry.
Experience in the mental health and community sector, with strong knowledge of services, block funded Mental health programs, NDIS and Understanding of Person-Centered Practices.
Basic skills in Microsoft Office, other common systems.
Current Police check - not older than 3 months
Valid NSW Drivers C class licence, with ability to drive vehicles in licence class.
Valid first aid certificate.
