Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.About the roleThe Connected Care Program aims to deliver better health outcomes and reduce hospitalisation rates by improved integration of care with existing GPs, providing specialist assessment, negotiated, comprehensive care plans and early escalation pathways for clients. The service provides a targeted and individualised service to consumers with complex chronic health or psychosocial needs at risk of recurrent ED presentations, admissions and readmissions. The service is primarily delivered in the home, with telehealth, phone or in-clinic appointments at Barwon Health facilities as required.The Barwon Health Connected Care Community Clinician will contribute to, and be part of the community-based, multidisciplinary team to support consumers and carers to achieve an optimum level of health independence through:
  • Comprehensive, co-ordinated and integrated multidisciplinary services
  • Provision of consumer-centred care to community members from a broad range of backgrounds
  • Contribute to the development and delivery of high-quality, cost-effective community-based Care Coordination services
  • Contribute to meeting the program key performance indicators (KPI's)
