
Health Care Worker - Aged Care

- Collaborate with the multidisciplinary team to achieve positive health outcomes for residents.
- Provide high-standard direct resident care as outlined in care plans under the supervision of the Registered Nurse and Enrolled Nurse.
- Assist residents with safe medication administration as directed by a Registered Nurse.
- Deliver resident-focused meal and food services, following the food service schedules and Grampians Health Food Safety Plan.
- Maintain a clean and safe environment for residents, visitors and staff by completing assigned cleaning and laundry duties, such as vacuuming, mopping, dusting and resident laundering.
- Accurately document resident care in accordance with health service guidelines and promptly report any incidents in the VHIMS system.
- Certificate III in Individual Support, Certificate IV in Ageing Support or equivalent.
- Assist Clients with Medication (HLTHPS006), preferred.
- Follow Basic Food Safety Practices (HLTFSE001).
- Adaptability, initiating action, collaboration, managing work (including time management), work standards and stress tolerance.
- Demonstrated person-centred care and respectful approach to patients, visitors and staff.
- Previous experience in a similar role would be preferred
