You’ll assess care needs, coordinate services, and ensure clients receive the best possible support tailored to their individual goals and government funding entitlements.With care and capability, you will:Provide person-centred case management using a consumer-directed approachConduct assessments and create initial care plans and budgets in collaboration with clientsSupport a seamless transition from enquiry through to care deliveryMonitor service delivery and proactively advocate for clients as their needs changeMaintain accurate documentation in Anglicare systems and ensure complianceFoster relationships with internal and external stakeholders to enhance growth and service reachRepresent Anglicare at events or presentations that promote our services and point of differenceWhat you’ll bringYou are a compassionate, detail-oriented case manager with experience in aged care and a passion for empowering older Australians.
