This vital role supports the Clinical Nurse Specialist in achieving service goals and providing exceptional, individualised care to Aboriginal patients and their families. The successful candidate will offer leadership, expert nursing knowledge, and facilitate care coordination, assessment, advice, and support to Aboriginal families, whilst promoting cultural awareness within the organisation. You will be responsible for engaging Aboriginal families in a clinical setting, acting as an advocate and collaborating with the healthcare team to improve health outcomes and family satisfaction.About the Service: Perth Children’s Hospital’s (Walking with Families) Ambulatory Care Coordination program provides out of hospital care to Aboriginal families who might find it hard to come into hospital for their paediatric appointments or who want to stay closer to home.About Perth Children’s Hospital (PCH): is Western Australia's specialist paediatric hospital and trauma centre, with 298 beds. - Proven ability to function effectively in a multidisciplinary team (Essential Criteria 5)
- A current resume which summarises your experience and includes both phone and email contact details of two professional referees, with at least one being a recent supervisor/line manager.
