Access theAbout the roleThis position will deliver intensive, clinical assessments, interventions and support to people who are at risk of frequent relapses in their mental health and recovery, whom without intensive clinical interventions, would result in them requiring frequent hospitalisations to an acute inpatient service.This position will work in collaboration with other community clinicians to reduce emergency department presentations, readmissions and overall lengths of stay for the identified consumer cohort.About youPlease refer to the relevant attached role description for the key capabilities, knowledge and experience requirements.Clinical Nurse Consultant NG7: $132,562 - $143,924 per annumHealth Practitioner HP4: $128,169 - $137,918 per annumMandatory Requirements:
