Tired of working on the frontline? Looking to join a team coordinating care and services to support patients in their own home? We have a opportunity available to work within the SA Community Care (SACC) and the My Home Hospital (MHH) program.Using your clinical skills, you will ensure a comprehensive client centred management plan is achieved for our clients together with:
- Conducting clients initial phone assessment, care planning and goal setting
- Coordination of in-home services to support clients receiving care at home
- Building and maintaining effective relationships with a variety of different stakeholders • Monitoring patient progress and updating care plans as appropriate
- Liaison with healthcare professionals and specialists
- Discharge planning including referrals to long term services
- Provision of telephone support, education, and advice
- 3-5 years' experience acute hospital experience as a Registered Nurse with unrestricted AHPRA registration • Recent experience in care coordination (highly desirable)
- Strong customer service orientation and relationship management skills
- Excellent clinical reasoning skills
- Ability to work in a fast-paced environment.
- Proficient computer skills to enable fast and accurate data entry into patient electronic medical records.
