About the RoleOPS-4 The Central Adelaide Local Health Network (CALHN) is seeking a dedicated and passionate Aboriginal and Torres Strait Islander Health Practitioner to join our Acute and Urgent Care program at The Queen Elizabeth Hospital Emergency Department. This role offers a unique opportunity to make a meaningful impact on the health and wellbeing of Aboriginal patients by providing culturally safe, holistic, and patient-centred care. Working within a multidisciplinary team, you will play a critical role in embedding an 'Aboriginal lens' into acute healthcare settings, ensuring Aboriginal patients receive the care and support they need to achieve better health outcomes. Your expertise will contribute to enhancing cultural safety, reducing the risk of disengagement, and improving continuity of care through appropriate referrals, discharge planning, and ongoing support.As a key member of CALHN's Aboriginal and Torres Strait Islander Health and Wellbeing Hub, you will provide clinical interventions, assessments, and care planning within your scope of practice, helping to improve access to quality healthcare services. Your role will directly support efforts to close the gap in Indigenous health outcomes by reducing hospital avoidance, supporting timely and effective care, and promoting health equity. Based within The Queen Elizabeth Hospital Emergency Department, this role will allow you to develop your skills in providing culturally safe and holistic care to Aboriginal patients in acute healthcare settings. Under the guidance of experienced clinicians, you will support patients presenting to hospital, attending outpatients, or engaging with other care programs by ensuring their healthcare journey is culturally appropriate, supportive, and well-coordinated. Your work will help strengthen patient engagement, improve discharge planning, and contribute to better health outcomes for Aboriginal communities.As a valued member of our team, you will receive comprehensive training and mentorship to build your clinical knowledge and practical experience.
