This is an excellent opportunity for a professional who is passionate about supporting individuals and families to achieve their goals.About the RoleAs a Disability Services Planner, you will be responsible for managing a portfolio of client records, ensuring high-quality and timely support is delivered. Your primary duties will include:
- Developing and reviewing comprehensive care plans based on individual needs and goals.
- Managing and prioritizing a demanding workload to meet key service delivery targets.
- Maintaining accurate and detailed client records using IT and data management systems.
- Engaging with a range of internal and external stakeholders to facilitate positive outcomes.
- Utilising your conflict resolution skills to manage difficult conversations and complex situations with tact and empathy.
- Conducting data analysis to inform decisions and contribute to service improvement.
- Proven experience in the disability or broader community services sector.
- Exceptional communication skills, both written and verbal, with a talent for stakeholder engagement.
- Strong IT competency and the ability to work effectively with client record systems and data.
- The ability to work both independently and as part of a dynamic team.
- A proactive approach to problem-solving and a high level of attention to detail.
