Band 4 salary, commencing at $38.73 per hour plus 12% superannuation (dependent on skills and experience)
The OpportunityWork with Council's dedicated and supportive Young, Inclusive & Ageing Communities department in delivering high-quality consumer-directed care. As Care Coordination Officer, you will provide vital administrative and coordination support to Care Partners responsible for care planning and support of older adults and people with disabilities in receipt of services under the Commonwealth Home Support Program (CHSP). The role is essential to facilitating the timely delivery of care services for older adults and people with disabilities, and high-quality care. Ensure all internal and external customer enquiries are responded to in a timely and customer focused manner.
Prepare and send client correspondence including but not limited to; client information pack; client care plan, agreement, and service information.
Assist Care Partner with the coordination and commencement of clients' support services including liaison with internal home and community care teams.
Coordinate changes required to clients' regularly scheduled services.
Escalate client incidents, feedback, and complaints (i.e. service dissatisfaction and change in care needs) to client's allocated Care Partner
Undertake rostering of Care Partners to enhance service efficiencies and client monitoring processes.
Maintain accurate and secure assessment, referral and care and service planning records, input of case notes, client files, and ensure documentation is recorded accurately, up to date in the Aged and Disability Services' Client Management System (CMS), the My Aged Care portal, and organisational financial and debtor systems
Participate in the development of an efficient, effective, and customer-focused care management team that operates as a business unit with a strong community focus.
Undertake other duties as directed by the Team Leader Care Partners and Coordinator of Assessment and Care Planning within the scope of this position.
Participate in the emergency relief efforts to support recovery during a municipal emergency/ event.
Key Requirements * Qualification in Business Administration (or similar) and/or demonstrated work experience in office administration with relevant experience.
Understanding of Aged and Disability Services administration within a local government or similar environment.
Demonstrated understanding of the needs of older people, people with disability, their carers, and people from diverse communities.
Demonstrated competence and experience in Microsoft Office software, including proficiency with Word and Excel.
Experience in the use of an Aged and Disability Services client management system would be an advantage or the demonstrated ability to learn and implement new computer software systems in a reasonably short time frame.
Demonstrated highly developed interpersonal skills and ability to respond to difficult situations appropriately.
Demonstrated excellent verbal communication skills particularly in relation to use of the telephone.
